Endometriosis
Endometriosis is a long-term condition in which tissue similar to the lining of the womb grows outside it, most often on the ovaries, fallopian tubes and the lining of the pelvis. This tissue responds to monthly hormone changes, causing inflammation, pain and scar tissue (adhesions). Cysts called endometriomas, or 'chocolate cysts', can also form on the ovaries. Endometriosis affects women of reproductive age and often starts in the teens or twenties. The main symptom is period pain severe enough to disrupt school, work or daily life, which is often dismissed as normal. Other symptoms include pain during or after sex, pain on opening the bowels or passing urine during periods, heavy periods and difficulty getting pregnant. The severity of symptoms does not always match the amount of endometriosis present. There is no permanent cure, but pain relief, hormonal treatment and keyhole (laparoscopic) surgery can control symptoms and help with fertility.

- Exact cause is not fully understood
- Retrograde menstruation (menstrual blood flowing back through the tubes into the pelvis)

- Starting periods early or having short menstrual cycles
- Immune and hormonal factors
How Endometriosis is diagnosed
- Pelvic examination
- Transvaginal ultrasound to look for endometriomas
- MRI of the pelvis for deep endometriosis
- Laparoscopy (keyhole surgery), sometimes with biopsy, to confirm the diagnosis

Is severe period pain normal?
Some cramping is common, but pain that stops you going to work or college, or that does not respond to regular painkillers, is not normal. It should be assessed by a gynaecologist, as endometriosis is one possible cause.
Can I get pregnant if I have endometriosis?
Many women with endometriosis conceive naturally, but it can reduce fertility in some. If you have been trying for a year without success, or six months if you are over 35, see a gynaecologist or fertility specialist.
Can endometriosis come back after surgery?
Yes, it can return, especially if hormonal treatment is not continued. Your doctor may suggest long-term medical treatment after surgery to reduce this chance.
Does pregnancy or menopause cure endometriosis?
Symptoms often ease during pregnancy and after menopause, when hormone levels fall, but this is not a reliable cure. Pregnancy should not be planned as a treatment for endometriosis.
