Ovarian Cyst
An ovarian cyst is a fluid-filled sac in or on an ovary. Most are 'functional' cysts that form as part of the normal menstrual cycle, when an egg follicle does not release its egg or the structure left after ovulation fills with fluid. These are common in women of reproductive age, usually cause no symptoms and often disappear on their own within a few menstrual cycles. Other types include endometriomas linked to endometriosis, dermoid cysts and cystadenomas. A small number of ovarian cysts, especially those found after menopause, need careful assessment to rule out cancer. Larger cysts can cause pelvic pain, bloating, a feeling of fullness or pain during sex. Sometimes a cyst bursts or twists the ovary (ovarian torsion), causing sudden severe pain that needs emergency treatment. Management ranges from repeat ultrasound monitoring to keyhole (laparoscopic) surgery, depending on the cyst's size and appearance and your age.

Causes and risk factors of Ovarian Cyst
- Normal ovulation (functional cysts)
- Endometriosis (endometriomas)
- Early pregnancy (corpus luteum cyst)
- Pelvic infection spreading to the ovaries
- Fertility treatment that stimulates the ovaries
- Non-cancerous or, less often, cancerous growths of ovarian tissue
How Ovarian Cyst is diagnosed
- Pelvic ultrasound (abdominal or transvaginal)
- Repeat ultrasound after a few cycles to see if the cyst has gone
- CA-125 blood test in selected cases, especially after menopause
- Pregnancy test to rule out ectopic pregnancy
- MRI if the ultrasound picture is unclear

Ovarian Cyst FAQs
Is an ovarian cyst the same as PCOS?
No. PCOS is a hormonal condition in which the ovaries contain many small follicles, together with irregular periods or raised male hormones. A single ovarian cyst is a separate finding that can occur in anyone who ovulates.
Will my ovarian cyst need surgery?
Many simple cysts go away without treatment and only need a follow-up scan. Surgery may be advised if a cyst is large, persistent, causing symptoms, or has features that need a closer look.
Can an ovarian cyst affect fertility?
Most functional cysts do not affect fertility. Cysts linked to endometriosis may, and any surgery on the ovary should be planned carefully to protect healthy tissue.
When is an ovarian cyst an emergency?
Go to an emergency department for sudden, severe pelvic or abdominal pain, especially with vomiting, fever, dizziness or fainting. This may mean a cyst has burst or the ovary has twisted, which needs urgent care.
