Losing Weight Without Trying — What to Rule Out

Also described as sudden weight loss, unintentional weight loss, losing weight fast.

Losing more than 5% of body weight over six to twelve months without trying — about 3.5 kg in a 70 kg adult — warrants investigation. Diabetes, thyroid disease, tuberculosis, depression and cancer all present this way, and basic blood tests with a chest X-ray identify most causes.

What causes unexplained weight loss?

Uncontrolled diabetes

Weight loss with thirst and frequent urination, reversible once treated.

Thyroid overactivity

Weight loss with rapid pulse, tremor, heat intolerance and anxiety.

Tuberculosis

Weight loss with evening fever, night sweats and cough. Common in India and fully curable.

Cancer

Several cancers present with weight loss before anything else, which is why a basic screen is arranged early.

Depression or malabsorption

Reduced intake, or coeliac disease and chronic gut conditions impairing absorption.

How it is investigated

  • ✓Full blood count, ESR, blood sugar and HbA1c
  • ✓Thyroid, liver and kidney function
  • ✓Chest X-ray and sputum testing for tuberculosis
  • ✓Abdominal ultrasound, endoscopy or CT depending on findings

Not every patient needs every test. The consultation decides which of these apply to you.

Frequently asked questions

How much weight loss should worry me?▼

More than 5% of body weight in six to twelve months without dieting or increased activity. In a 70 kg adult that is around 3.5 kg. Faster loss, or loss alongside fever, cough, a lump or bleeding, warrants earlier assessment.

Does weight loss mean cancer?▼

Not usually. Diabetes, thyroid disease, tuberculosis and depression are commoner explanations. But cancer is among the possibilities, and it is far more treatable when found at this stage, which is the reason for investigating rather than waiting.
