Leg Pain When Walking — Causes and Circulation Checks
Also described as calf pain on walking, claudication, leg cramps while walking.
Cramping calf or thigh pain that appears after a predictable walking distance and eases within minutes of stopping suggests narrowed leg arteries. It is a marker of arterial disease elsewhere, including the heart, so it is worth investigating even when the pain itself is mild.
🚨 Get emergency care if you have
- •Leg pain at rest or at night, relieved by hanging the leg down
- •A wound or ulcer on the foot that is not healing
- •A cold, pale or blue foot — this is an emergency
- •Blackened skin on a toe
- •Sudden severe leg pain with loss of sensation or movement
What causes leg pain on walking?
Peripheral arterial disease
Narrowed leg arteries from atherosclerosis. Strongly associated with smoking, diabetes and high cholesterol.
Spinal stenosis
Leg pain on walking that eases on bending forward or sitting, rather than simply stopping.
Diabetic neuropathy
Burning or tingling pain, often worse at night and not clearly related to walking distance.
Varicose veins
Aching and heaviness that worsens through the day and improves with elevation.
Muscle cramps
Sudden, brief and unrelated to a consistent walking distance.
How it is investigated
- ✓Ankle-brachial pressure index — a simple bedside circulation test
- ✓Doppler ultrasound of the leg arteries
- ✓CT or MR angiography where intervention is being considered
- ✓Blood sugar, lipid profile and kidney function
Not every patient needs every test. The consultation decides which of these apply to you.
Frequently asked questions
How do I know if leg pain is circulation or a nerve problem?▼
Circulation pain is reproducible — it appears at roughly the same walking distance and eases within a few minutes of standing still. Nerve or spinal pain is more variable, often improves on leaning forward or sitting, and may come with numbness. An ankle-brachial index takes minutes and separates the two.
Can this be treated without surgery?▼
Often yes. Stopping smoking, supervised walking exercise, and treatment of diabetes and cholesterol improve walking distance substantially. Angioplasty or bypass is reserved for pain at rest, non-healing wounds, or symptoms that remain disabling despite these measures.
