Swollen Joints — Causes and Urgent Warning Signs

Also described as swollen joint, inflamed joint, arthritis swelling.

A single hot, red, exquisitely painful joint with fever must be treated as a possible joint infection and assessed the same day, because untreated it destroys cartilage within days. Gout produces a similar picture. Swelling across many joints with morning stiffness suggests inflammatory arthritis.

🚨 Get emergency care if you have

  • •One hot, red, very painful joint with fever — same-day assessment
  • •Inability to move the joint at all
  • •Joint swelling after a penetrating injury
  • •Swelling with a spreading red rash
  • •Swelling with unexplained weight loss or night sweats

What causes joint swelling?

Septic arthritis

Joint infection. A surgical emergency — delay causes permanent joint damage.

Gout

Sudden severe pain, often the big toe, with redness. Confirmed on joint fluid or uric acid.

Rheumatoid arthritis

Symmetrical small-joint swelling with morning stiffness over 30 minutes.

Osteoarthritis

Bony swelling with activity-related pain, typically knees, hips and finger joints.

Injury with bleeding into the joint

Rapid swelling within an hour of trauma, suggesting a ligament tear or fracture.

How it is investigated

  • ✓Joint fluid aspiration and analysis — the decisive test for infection and gout
  • ✓CRP, ESR and white cell count
  • ✓Rheumatoid factor and anti-CCP where inflammatory arthritis is suspected
  • ✓X-ray or ultrasound of the joint

Not every patient needs every test. The consultation decides which of these apply to you.

Frequently asked questions

How do I know if a swollen joint is infected?▼

Infection typically affects one joint, which becomes hot, red and so painful that almost any movement is refused, often with fever and feeling unwell. Gout can look identical, so the fluid is aspirated and examined. Because untreated infection destroys the joint within days, this combination is assessed the same day.

Is gout caused only by diet?▼

Diet contributes, but most uric acid is produced by the body, and kidney function, genetics, diuretics and alcohol matter as much as red meat or seafood. Recurrent attacks are treated with uric-acid-lowering medication, not diet alone.

Other symptoms

This page is general health information, not a diagnosis or a substitute for a consultation. Symptoms overlap between harmless and serious conditions, and only an examination can tell them apart. If you are worried, see a doctor.

Plan your first visit

Not sure which specialist to see? Tell our care team the reason for your visit and we will help you choose.

  • Bring previous prescriptions, relevant test reports or scan images, a medication list and photo ID.
  • If you plan to use insurance, bring your policy details. OPD, tests and admission benefits depend on your individual plan and approval.
  • Your first consultation determines which tests or treatments are needed; not everyone needs every test listed on this page.
