Difficulty Passing Urine — Causes in Men and Women

Also described as weak urine stream, straining to urinate, poor flow, urinary hesitancy.

A weak stream, hesitancy and getting up at night are most often caused by an enlarged prostate in men over 50, and are very treatable. Complete inability to pass urine with a painful lower abdomen is acute retention and needs emergency care immediately.

What causes difficulty passing urine?

Benign prostatic enlargement

Weak stream, hesitancy, incomplete emptying and nocturia in men over 50.

Urethral stricture

Narrowing after infection, catheterisation or injury, causing a thin, slow stream.

Prostate cancer

May cause similar symptoms. Assessed with examination and PSA.

Bladder or pelvic floor dysfunction

Incomplete emptying without obstruction, in men and women.

Medication

Some antihistamines, decongestants and antidepressants precipitate retention.

How it is investigated

  • ✓Uroflowmetry and post-void residual ultrasound
  • ✓PSA and digital rectal examination in men
  • ✓Urine culture and kidney function
  • ✓Cystoscopy where a stricture is suspected

Not every patient needs every test. The consultation decides which of these apply to you.

Frequently asked questions

Does an enlarged prostate mean cancer?▼

No. Benign enlargement is extremely common with age and is not cancer, nor does it become cancer. The symptoms overlap, which is why assessment includes a PSA test and examination — but most men with these symptoms have a benign cause and respond well to medication.

What is acute urinary retention?▼

Being unable to pass urine at all while the bladder fills and becomes painful. It is an emergency — the bladder is drained with a catheter, which brings immediate relief and protects the kidneys. Do not wait it out overnight.

  • Bring previous prescriptions, relevant test reports or scan images, a medication list and photo ID.
  • If you plan to use insurance, bring your policy details. OPD, tests and admission benefits depend on your individual plan and approval.
  • Your first consultation determines which tests or treatments are needed; not everyone needs every test listed on this page.
