Coughing Up Blood — Why It Always Needs Checking
Also described as blood in sputum, haemoptysis, blood when coughing.
Coughing blood always needs assessment, even when it is a single streak in phlegm. In India, tuberculosis is a leading cause and is curable when treated. Large volumes of blood, or blood with breathlessness or chest pain, need emergency care at Iswarya Hospital, OMR Chennai.
🚨 Get emergency care if you have
- •Coughing more than a few teaspoons of blood — go to an emergency department

- •Blood with fever, night sweats and weight loss
- •Any amount of blood in a smoker over 40

What causes coughing blood?
Tuberculosis
Cough over two weeks with fever, night sweats and weight loss. Curable with a full treatment course.
Chest infection or bronchitis
Streaks of blood in sputum during an acute infection, resolving as it clears.
Bronchiectasis
Long-standing daily sputum production with recurrent infections and intermittent bleeding.
Lung cancer
Persistent cough with blood, weight loss and chest pain, usually with a smoking history.
Nose or throat bleeding
Blood from the upper airway that is coughed out, mimicking a lung source.
How it is investigated
- ✓Chest X-ray, usually the same day
- ✓Sputum examination for tuberculosis
- ✓CT chest where the X-ray is normal but bleeding persists
- ✓Bronchoscopy for direct airway inspection
Not every patient needs every test. The consultation decides which of these apply to you.
Frequently asked questions
Is a small streak of blood serious?▼
It may not be, but it cannot be dismissed without a chest X-ray and, where the cough has lasted over two weeks, a sputum test for tuberculosis. Small amounts frequently come from infection, and treating that is straightforward — but the same appearance can be the first sign of something that is far better found early.
Could this be tuberculosis?▼
It is a realistic possibility in India, particularly with a cough lasting more than two weeks alongside evening fever, night sweats and weight loss. Sputum testing gives an answer quickly, and TB treatment is free at government centres and fully curable when completed.
